Appointments

Convenient appointment times are available. Plan on spending 1-2 hours with us on your first visit. In order for your evaluation to be as thorough as possible, please bring:

Your past medical records
An accurate list of medications (including doses)
Recent CAT or MRI films (and any written reports)
Your written questions and concerns, including if possible, a time log of your daily symptoms
Name and address of your primary doctor.
A family member of friend
Eye glasses and hearing aids if necessary
