  • When you call to schedule your initial appointment, you will be asked to provide insurance information and the name of a referring or primary care physician.
  • Please arrive on time for your appointment. Depending on your records, you may need to sign authorization forms for disclosure of medical information. Please bring any test results, scans, or physician notes that you feel are important.
  • Please bring an updated list of your medications, dosages, and times. If you do not have a list, then bring the bottles. We will want to verify the medications against your medical record.
